Player Releases

If you need to obtain a release in order for your child or ward to try out for or be added to a roster of a team that is part of another organization, please print, complete, and follow the instructions on the player release form available here. For the fastest turnaround time, please send your completed form by email to all of the following:

• the association co-presidents at co-presidents@pelhamhockey.com;
• the association treasurer at treasurer@pelhamhockey.com; and
• the association registrar at registrar@pelhamhockey.com.
